CEDAR HAMMOCK FIRE CONTROL DISTRICT
REQUEST FOR PROPOSALS
This is a request to submit proposals for Insurance Benefits Broker. A complete description of the coverage being requested will be available at the Cedar Hammock Fire Control District’s (“District”) business office, located at Cedar Hammock Fire Department – Station #1, 5200 26th Street West, Bradenton, Florida 34207, Monday through Thursday, between the hours of 7:00 a.m. and 5:00 p.m., and on the District’s website at: www.chfr.org.
Proposals must be submitted by mail or hand-delivery to the District’s business office by November 13, 2018, at 2:00 p.m. and shall be delivered in a sealed envelope with the words “GROUP INSURANCE BENEFITS PROVIDER PROPOSAL” clearly marked on the front. On November 13, 2018, at 3:00 p.m. the bids will be publicly opened at the District’s business office. On November 14, 2018, at 9:00 a.m. and, if required, November 15, 2018, at 9:00 a.m., a Selection Committee will convene a publicly noticed meeting, held at the District’s business office, for purposes of ranking and making a recommendation to the Cedar Hammock Fire Control District’s Board of Fire Commissioners (“Fire Commission”). The Selection Committee’s recommendation will be presented to the Fire Commission for formal action at a regularly scheduled public meeting, held at Cedar Hammock Fire Department – Station #1, 5200 26th Street West, Bradenton, Florida 34207, on Wednesday, November 15, 2018, at 6:00 p.m. The Fire Commission reserves the right to reject any or all proposals, as well as the right to reject any portion of any proposals.

Architectural and Engineering Services
The Cedar Hammock Fire Rescue District seeks Statements of Qualifications from firms for architectural and engineering services to develop construction drawings for a fire training tower, to include relocation of gas piping mock-up, demolition of existing wooden tower, and replacement of decking, railing support, and stairs over current confined space mock-up.
